Practice Policies & Legal Disclosures

1. Physician & Practice Identification

Ibn Sina Physicians, PLLC is a physician-owned and physician-led medical practice. We are dedicated to providing clinician-guided telehealth care focused on long-term safety and outcomes. Our practice is registered as a Professional Limited Liability Company (PLLC).

2. Telehealth Informed Consent

Telehealth involves the delivery of healthcare services using interactive audio and video technology. By using our services, you consent to receive care via telehealth and acknowledge that while it offers accessibility, it is limited by the lack of an in-person physical examination.

3. Emergency & Limitations of Care

IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR GO TO THE NEAREST EMERGENCY DEPARTMENT. Telehealth is for non-emergent care only. Ibn Sina Physicians does not provide emergency treatment or acute crisis services.

4. Served & Licensure

Telehealth services are provided only in states where the physician is currently licensed. Availability may change over time, and patient location is verified at the start of each visit.

5. Prescribing & Controlled Substances

We do not prescribe controlled substances (Schedule II–V) through our telehealth practice. Prescription decisions are made case-by-case based on clinical appropriateness and applicable federal and state laws.

6. Payment, Refund & Cancellation Policy

Ibn Sina Physicians is a cash-pay practice and does not bill insurance. Payment is required at the time of scheduling. Cancellations with less than 24 hours' notice may be subject to a fee. Clinical visits are non-refundable once started.

7. HIPAA Privacy Notice (Notice of Privacy Practices)

Effective Date: May 20, 2024. This Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. We are required by law to maintain the privacy and security of your protected health information (PHI).

How We May Use and Disclose Your Health Information
  • Treatment: We may share your information with other professionals who are treating you.
  • Payment: We can use and share your information to bill and get payment.
  • Health Care Operations: We use and share your info to run our practice and contact you.
Your Rights Regarding Your Information
  • Access: You have the right to inspect and receive a copy of your records.
  • Amendments: You can ask us to correct information you think is incorrect.
  • Accounting: You can request a list of certain disclosures we have made.
  • Restrictions: You can ask us not to use or share certain info for payment or operations.
  • Confidential Comms: You can request specific communication methods or locations.
How to Exercise Your Rights

To exercise any of the rights listed above or to request copies of your health records, please submit a written request to our Privacy Officer at the email provided below.

Filing a Complaint & No Retaliation

If you feel your privacy rights have been violated, you may file a complaint calling our office or the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

Breach Notification

We will notify you promptly in the event of a breach that may have compromised the privacy or security of your unsecured protected health information.

8. Scope of Services & Clinical Judgment

Our services prioritize physician judgment and long-term care management. We do not provide one-off administrative sign-offs or algorithmic prescribing without thorough clinical evaluation.
