Practice Policies & Legal Disclosures

01. Physician & Practice Identification

Ibn Sina Physicians, PLLC is a physician-owned and physician-led telehealth practice. Registered as a Professional Limited Liability Company, our group is dedicated to providing high-quality medical care under the direct supervision of board-certified physicians. We operate as a cash-pay entity, prioritizing clinical integrity over administrative volume.

02. States Served & Licensure Disclosure

Telehealth visits are available only to patients physically located in: New York (NY), Florida (FL), Texas (TX), Alaska (AK), and Kansas (KS) at the time of the visit. Patient location is verified at the start of each visit.

03. Emergency & Limitations of Care

Ibn Sina Physicians does NOT provide emergency medical services. If you are experiencing a life-threatening emergency, you must immediately call 911 or visit the nearest emergency department. Our platform is for non-acute, scheduled care only. Patients must not rely on clinical messaging for life-threatening concerns.

04. Telehealth Informed Consent

Telemedicine involves the delivery of healthcare services using interactive technology. By using our platform, you acknowledge the inherent risks and benefits of remote care and understand that while telehealth provides convenience, it is limited by the absence of a physical examination. Clinical judgment may require an in-person follow-up with a local provider.

05. Payment, Refund & Cancellation Policy

We operate as a cash-pay practice and do not bill insurance or federal health programs. Payment is required at the time of scheduling. Cancellations must be made at least 24 hours in advance to be eligible for a refund. No-shows or late cancellations result in visit fee forfeiture. Clinical consultations are for professional time and do not guarantee specific outcomes or prescriptions.

06. Prescribing & Controlled Substances Policy

We do not prescribe controlled substances (Schedule II–V) through our telehealth practice. Prescription decisions are made case-by-case based on clinical appropriateness and applicable federal and state laws.

07. HIPAA Notice of Privacy Practices (NPP)

This Notice describes how medical information about you may be used and disclosed and how you can get access to this information.

Uses and Disclosures: We may use or share your protected health information (PHI) for Treatment, Payment, and Healthcare Operations (quality improvement and business management).

  • Access & Copies: Right to inspect/receive copies of your medical records.
  • Amendments: Right to request corrections to health info.
  • Accounting: Right to request a list of certain PHI disclosures.
  • Restrictions: Right to ask us to limit info usage/sharing.
  • Confidentiality: Right to request specific contact methods.

Complaints & No Retaliation: File with us or HHS. We will not retaliate against you for filing a complaint.
