Telehealth Informed Consent
HIPAA Privacy Notice

By using our services, you consent to receive physician-led care via secure telehealth technology. You acknowledge the limitations of virtual evaluations and agree to the clinical judgment of our board-certified physicians regarding your suitability for telehealth treatment. This practice adheres to strict protocols for remote patient assessment.

Notice of Privacy Practices (NPP)
Effective Date: October 1, 2023

Responsibilities: We maintain the privacy of your health information (PHI) and provide you with this notice. We will notify you concisely in the event of a breach of unsecured PHI.

Uses & Disclosures:
• Treatment: We coordinate care with medical providers.
• Payment: We disclosure PHI for invoicing or records requested by patients for claims.
• Health Care Operations: We use PHI for clinical quality management.

Patient Rights:
• Access and copies of your clinical records.
• Amend information you believe is inaccurate.
• Accounting of PHI disclosures.
• Request specific restrictions on use or disclosure.
• Request confidential communications at a specific location.

Exercise Rights & Complaints: Contact our Privacy Officer at Ibn Sina Physicians, PLLC or via contact@ibnsinaphysicians.com to request records or file a grievance. Complaints can also be filed with the HHS. We maintain a strict no-retaliation policy.

Terms of Service
States Served & Licensure

These terms govern your use of the Ibn Sina Physicians platform. A physician-patient relationship is only established after a formal clinical evaluation. Our board-certified staff reserves the right to decline care if a patient's needs exceed the scope of telehealth.

Telehealth visits are available only to patients physically located in: New York (NY), Florida (FL), Texas (TX), Alaska (AK), and Kansas (KS) at the time of the visit. Patient location is verified at the start of each visit.

Emergency & Limitations of Care
Payment & Refund Policy

Telehealth is for non-emergent care only. If you are experiencing a medical or psychiatric emergency, you must call 911 or go to the nearest emergency department immediately. We do not provide acute crisis intervention through this platform.

We are a cash-pay practice and do not bill insurance. Payments are collected at the time of scheduling to reserve your visit. Cancellations made within 24 hours of the appointment may be subject to a fee. Refunds are issued on a case-by-case basis before care is rendered.

Prescribing & Controlled Substances
Scope of Services

We do not prescribe controlled substances (Schedule II–V) through our telehealth practice. Prescription decisions are made case-by-case based on clinical appropriateness and applicable federal and state laws.

Our practice focuses on physician-led primary care consultations, chronic disease monitoring, and clinical guidance. We facilitate specialist navigation and long-term care plans based on evidence-based protocols and physician clinical judgment.
