Telehealth Informed Consent

Telehealth involves the use of electronic communications to enable healthcare providers at Ibn Sina Physicians, PLLC to provide clinical services. By initiating care, you acknowledge the benefits and risks associated with virtual consultations, including individual variations in technology and clinical suitability for remote diagnosis.

Notice of Privacy Practices (NPP)

Effective Date: January 1, 2024. This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Ibn Sina Physicians uses your Protected Health Information (PHI) primarily for treatment, payment, and health care operations.

  • Treatment: Providing and coordinating clinical care.
  • Payment: Processing cash-pay fees for clinical visits.
  • Health Care Operations: Internal quality assessments and clinical business planning.
  • Access/Copies: You have the right to request access to records via our secure portal.
  • Amendments: You may request amendments to PHI you believe is inaccurate.
  • Accounting: You may request an accounting of certain disclosures.
  • Restrictions: You may request restrictions on specific PHI uses/disclosures.
  • Confidentiality: You have the right to request private communication channels.

To exercise your rights, contact our Privacy Officer via the practice clinical email or mailing address. You may file a complaint with us or the Secretary of HHS if you believe privacy rights are violated; there is no retaliation for doing so. We will notify you following any breach of PHI.

Terms of Service

Ibn Sina Physicians, PLLC offers physician-led telehealth services. Use of the website and portal implies acceptance of these Terms. No physician-patient relationship is established until a formal clinical visit is initiated and confirmed by our medical team. Governing law for this practice resides within the jurisdictions of NY, FL, TX, AK, and KS.

States Served & Licensure Disclosure

Telehealth visits are available only to patients physically located in: New York (NY), Florida (FL), Texas (TX), Alaska (AK), and Kansas (KS) at the time of the visit. Patient location is verified at the start of each visit.

Emergency & Limitations of Care

Telehealth is for non-emergent care only. For urgent or emergent symptoms, patients must call 911 or go to the nearest emergency department immediately. Ibn Sina Physicians does not provide acute crisis intervention or emergency hospital-based medicine and does not manage life-threatening conditions online.

Payment, Refund & Cancellation Policy

Ibn Sina Physicians is a cash-pay PLLC. We do not participate in insurance billing. All fees are transparent and due at the time of booking. Cancellation with more than 24-hour notice allows for a partial refund; no-shows and late cancellations are non-refundable to account for pre-allocated physician time.

Prescribing & Controlled Substances Policy

We do not prescribe controlled substances (Schedule II–V) through our telehealth practice. Prescription decisions are made case-by-case based on clinical appropriateness and applicable federal and state laws.

Scope of Services

Our clinical scope focuses on physician-led chronic disease management, general non-urgent medical concerns, and wellness consultations. We do not provide long-term pain management, disability evaluations, or surgical coordination through our primary telehealth platform.

Physician & Practice Identification

Ibn Sina Physicians, PLLC is a physician-owned professional medical corporation. Every consultation is performed by a licensed physician board-certified in their specialty. We prioritize physician-led continuity over algorithmic volume-based telehealth models.

Accessibility Statement (ADA)

Ibn Sina Physicians is committed to making our digital telehealth services accessible to all individuals. We regularly improve our user interface to align with ADA requirements. If you encounter digital barriers, please contact our support team for assistance.
