Ibn Sina Physicians, PLLC is a physician-owned, patient-centered telehealth practice. We are committed to clinical excellence, transparency, and compliance. The following policies outline the terms of our professional relationship and the standards of care you can expect from our providers.
1. Telehealth Informed Consent
By utilizing Ibn Sina Physicians, you acknowledge that telehealth involves the delivery of healthcare services using electronic communications. While telehealth offers convenience, you understand the risks include potential technical failures or security breaches, despite our use of encrypted platforms. You have the right to withhold or withdraw consent to telehealth at any time without affecting your right to future care or treatment outside of this practice.
2. HIPAA Notice of Privacy Practices (NPP)
Effective Date: January 1, 2026
This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Ibn Sina Physicians, PLLC utilizes the DrChrono Electronic Health Record (EHR) system to securely store and process your Protected Health Information (PHI). We will not disclose your PHI without your written authorization, except as permitted by law.
How We Use and Disclose Protected Health Information (PHI):
- Treatment: To provide, coordinate, or manage your health care and any related services.
- Payment: Although we are a cash-pay practice, we may use PHI to process payments or provide documentation for your own reimbursement requests.
- Health Care Operations: For business activities such as quality assurance, clinician review, and administrative management.
Your Patient Rights Regarding Your PHI:
- Access/Copies: You have the right to inspect and obtain electronic or paper copies of your health record.
- Amendments: You can ask us to correct health information that you think is incorrect or incomplete.
- Accounting of Disclosures: You may request a list of certain times we have shared your health information.
- Restrictions: You have the right to ask us to limit how we use or share your information.
- Confidential Communications: You have the right to request specific methods or locations for our communications.
How to Exercise These Rights: To exercise rights or request records, please submit a written request via our secure portal.

3. Terms of Service
This website (ibnsinaphysicians.com) is provided for informational and scheduling purposes. By accessing our site, you agree to comply with our professional requirements and administrative protocols. We reserve the right to terminate access for any user who violates practice guidelines or engages in fraudulent activity.
4. States Served & Licensure Disclosures
Telehealth visits are available only to patients physically located in: New York (NY), Florida (FL), Texas (TX), Alaska (AK), and Kansas (KS) at the time of the visit. Patient location is verified at the start of each visit.
5. Emergency & Limitations of Care
Ibn Sina Physicians does not provide emergency medical services. If you are experiencing a life-threatening emergency, call 911 or go to the nearest emergency department immediately. Telehealth is not a replacement for in-person emergency care or comprehensive primary care. We do not guarantee specific outcomes or treatment success.
6. Payment, Refund & Cancellation Policy
We are a cash-pay practice and do not bill insurance or participate in government programs (Medicare/Medicaid). Payment is due in full at the time of scheduling via DrChrono to confirm your appointment. Due to the professional time reserved for clinical review and consultations, cancellations made less than 24 hours before the appointment are non-refundable. Clinical fees cover the physician's expertise and evaluation, and do not guarantee a prescription.
7. Prescribing & Controlled Substances Policy
We do not prescribe controlled substances (Schedule II–V) through our telehealth practice. Prescription decisions are made case-by-case based on clinical appropriateness and applicable federal and state laws.
8. Scope of Services
Our scope is limited to non-emergent, physician-led telehealth consultations. We focus on acute episodic care and specific clinical protocols. We do not provide long-term primary care, pediatric emergency services, or management of unstable chronic conditions that require hospital-based resources.
9. Physician & Practice Identification
Ibn Sina Physicians, PLLC is a professional limited liability company owned and operated by licensed physicians. All clinical services are supervised or provided by board-certified physicians. Our administrative offices are focused on multi-state telehealth coordination. Clinical inquiries should be directed through the secure DrChrono portal.
